truForm: Security & Settings

    1. MySecurePractice truForm allows you to receive immediate text message or email alerts by selecting the alerts you would like to receive below.

      Message frequency is based on usage. PBHS does not have a separate charge for this service; however, message and data rates may apply from your mobile carrier.

      This notification setting will alert you once any user submits your online truForm: 

      1. On the My Profile page
      2. Select the truForm tab

      3. Under the Activity Preferences section:

      4. Under the drop down for  select the notification option you would like:

      5. Once your changes are complete, select the Save Profile button (located in the top/right hand corner):

    2. Each truForm invitation will include a one-time Quick Access link, which allows patients to access their assigned forms without needing to register an account with MySecurePractice. The Quick Access link will expire if not used within the set period of time.

      1. On the My Profile page (COMING SOON)
      2. Select the TRUFORM tab

      3. Under the drop down for  select the number of days you would like as your default:

  • Security

    Just like an online banking system, we use the following encryption to protect ALL patient data:

    • truForm is hosted on a secure server.
    • 256 bit encryption on the front and back-end.
    • We follow all HIPAA laws regarding passing patient information online.

    Understanding Health Information Privacy (HIPAA):

    The HIPAA Privacy Rule provides federal protections for personal health information held by covered entities and gives patients an array of rights with respect to that information. At the same time, the Privacy Rule is balanced so that it permits the disclosure of personal health information needed for patient care and other important purposes. The Security Rule specifies a series of administrative, physical, and technical safeguards for covered entities to use to assure the confidentiality, integrity, and availability of electronic protected health information.

    Once the online form is opened within your browser, you can locate the following security information:

    • If you are visiting the website with a secure connection, you will be able to identify the website through the site’s certificate. A secure or encrypted website address will begin with HTTPS rather than HTTP, and you will often see some sort of icon in the browser such as a padlock indicating that the website is secure. Secure connections use certificates to identify the website and to encrypt your connection.

    ill be updated periodically by PBHS to ensure the most up-to-date information is added. For detailed information about what updates have been completed, please see below:
